Level 1 - Basic Information
Provides access to your certificate of coverage and dental benefit summary.

We have an easy 2-step process that will allow you to access your basic plan information. First, create a UserID and password. Next, provide the primary insured member's name, date of birth, and zip code. Now you're connected to your account.

Level 2 - Protected Health Information
Once you have visited the dentist, you can access pending and paid claims, pretreatment estimates and dental or orthodontic maximums and deductibles.

To connect to your Protected Health Information, you'll need the member's Social Security Number/Member ID or the private source code that can only be found on a benefit statement (claim explanation) that you have received in the last 15 months.
